Accidental Insurance Program

The Conservation Centre of Herbs Yoga and Naturopathy an NGO is implementing Meningitis & Typhoid awareness and vaccination cum personal accidental insurance program to BPL and above group citizens in Delhi . This society is affiliated with Royal Society for Education, Environment and Rural Health Care, Lucknow , a micro finance agent of United India Insurance Company Limited (A Government of India undertaking). The organization will implement the following two schemes:

Yearly Membership Charge	Rs.200/-	Rs.100/-
Sum Insured to Members	Rs.1, 00,000/-on Unnatural Death	Rs.50000 Unnatural Death
Disability Cover	Rs.1,00,000 for above 50% Permanent Disability Rs.50,000/- For 50 % Disability	Rs.50,000 for above 50% Permanent Disability Rs.25,000/- for below 50 % Disability
Free Vaccine	Meningitis( Single Dose Haemophilus Influenzae Type-B, Cast around Rs.600/-) Age-01 to 05 years.	Typhoid ( Single Dose Cast around Rs.300/-)

Validity of insurance and membership subscription	One Year	One Year
Claim settlement period	Rs.1, 00,000/- Within 72 Hrs after summation of claim papers	Rs.50,000/- Within 24 Hrs after summation of claim papers.
Membership yearly subscription After one year.	Rs.100/- only to cover sum assured of Rs.1,00,000	Rs.50/- only to cover sum assured of Rs.50,000
Age group of Insurance	10-70 years.	10-70 years.

Documents required for final claim	Death Certificate issued by MCD, Gram Penchant or any Local authority. Hospital certificate reflecting percentage of disability.
